Is there a Dental Health Questionnaire to be filled out? It's mentioned in the Medical Director's letter but don't see it online.
Also, where does the Cardiology Screening Form go when completed? The form doesn't mention. White envelope?

Also, where do we find the The Cadet Information Form. I see the Cadet Contact Form but not the Cadet Information Form.

FWIW, last year, all medical forms (dental, cardiology and immunization) all went to this email address:
SMB-CGA-Clinic-Immunizations@uscg.mil

Last year, there was a nurse named Dawn Liddick handling this email account. She was extremely responsive, helpful and friendly!

I would send the forms you have to that email, and ask them any questions you have about the dental form, etc.

You may want to reach out to your AO about that. All the forms you need should be available in the portal or it looks like this year they mailed them to you.

There’s a chance this form has been eliminated, but the checklists haven’t been properly updated. The reason I think this could be the case is I know this year you guys started the background check/security clearance process early. If it’s anything like last year, all the info you needed for that was basically the same (and more) as what was needed for that Cadet Information form.

There’s a chance they’re trying to streamline the paperwork process a bit but are being a little clumsy about it.

I could be wrong on this, but the new superintendent has stressed all year about looking at everything they do and changing things that aren’t necessary or add value. There’s a lot of room for that in the onboarding process for sure!

Again, reach out to AO for definitive answers. Share your findings on the 2028 parents facebook group. You can share here too, but that FB group is a concentrated with those who need it most.
 
Good Morning! Has anyone gotten an answer concerning whether or not there is a Cadet Information Form that needs to be completed? We’ve completed every other document but this form isn’t online nor did it come via mail with the other forms.

My DS completed and returned what was sent. We decided if something is missing they will surely let him know.

We got hung up on getting the appointment acceptance back right away (which my son thought was the idea) and now as he's filling out forms from the website (so many forms from so many places, lol) they say to return in the "white envelope". Assuming that means the envelope he already sent back with his acceptance/birth cert/social security card. He currently has calls in to find out how we get a new "white envelope" or send on of our own and to whom.

We didn't use the provided white envelope last year and instead put everything in a priority envelope and mailed to Admissions Department at the USCGA campus address. That way we had tracking and verification it was received. Make sure to make a copy of every form and bring with you to Day 1.

Now apparently we've received a packet of forms at home by mail with a "white envelope", lol...so I guess this supersedes the ones online? Anyway, the mystery deepens....

I haven't been able to locate that document either. My son is reaching out. I'll post if I hear anything.

The ones that you receive in the packet slightly differ from the ones found online, so I'd recommend starting over with the ones in the packet (that's what we did at least lol). Cadet Information Form and Dental Health Questionnaire stumped us as well - the health office confirmed that the Dental Health Questionnaire is now no longer a required form and incoming cadets fill it out sometime over swab summer instead (but you can email the immunizations email address and ask for it if you'd like to get it done now).

The Cadet Information Form might also be the same deal this year, where it's no longer required and cadets do it over the summer.

I have to assume if no one has it, it's not going to be required (or they'll address it at some point prior to Swab Summer). Not worth sweating over!
 
We filled out the forms from online prior to receiving the enrollment forms via mail and had to redo several of them because we didn’t realize the signatures had to be in blue ink. That was also when the cadet handbook wasn’t accessible online, otherwise we’d have caught it by reading the handbook. So make sure to use blue for signing! After the enrollment forms are sent and confirmed received, we’re of the impression that we only have the surveys left, which as I understand will come a little later this summer. Passport came back fast, but if you still have that to get as well, I’d recommend expediting it and getting 2 day return shipping.
 
Regarding passports, because there are often questions here, go to the State Department website and look up processing times as they change often. That way you're not surprised when it takes so long to get it back.

If you need it faster than standard processing you can pay extra to expedite, and for faster mailing. But even expedited and with faster mailing you have to allow 3 weeks from when you mail the forms to get it back.

If my kids was waitlisted I wouldn't wait to get a passport for them. If you get the call a week out from the start of swab summer it's too late for them to be able to do any international stints on the Eagle. Worst case your kid has a passport so they can go on a foreign vacation.
 
So, I have a crazy question. My son is waitlisted for the class of 2028 and we realize that there is not much chance of him getting "the call," but we want to be ready just in case. On another thread, someone already shared a link for the class of 2028 Handbook. Would anyone be willing to share a pdf of the immunization form, cardiology form, and dental form? We would greatly appreciate it! :)
