Enrollment forms arrived in the mail today

There were a few subtle differences on a few of the forms. We had filled out a bunch already from the site but decided to start over with the pack they sent.
 
The enrollment forms package doesn't seem to include the Cadet Information Form, Emergency Contact Form, or any of the Medical Screening Forms (Immunizations, Cardiology, or Dental Health).

I'm able to just print out the Emergency Contact Form, Immunizations, and Cardiology easily enough from the website, but does anyone have any idea where to find the Cadet Information Form or the Dental Heahlth Questionnaire?

Both are still on the Class of 2028 checklist, but not on the enrollment forms page or in the package.

@mcboiler or @Eaglehurst What are your plans for this?
 
Good question - DS had prom last night so todays plans to complete paperwork have gone awry. Will likely print out and complete what available and if an update/addition is needed I am sure they will tell us.....

I am sure I saw the dental form on the enrollment forms page recently but I can't locate it now. If we find anything out through the process we will let you know.
 
Does anyone know if we need to obtain a 2024 w4 (the one online is 2023)?

The one enclosed in the package is the 2023 W4, so it should be okay 🤷‍♂️.

In the enrollment forms package there's a letter discussing it that says the questionnaire and surveys will be sent out via email 2-3 weeks prior to Day 1.
 
So, I have a crazy question. My son is waitlisted for the class of 2028 and we realize that there is not much chance of him getting "the call," but we want to be ready just in case. On another thread, someone already shared a link for the class of 2028 Handbook. Would anyone be willing to share a pdf of the immunization form, cardiology form, and dental form? We would greatly appreciate it! :)

All the forms are in the 2028 portal on the website.

Did anyone get an email confirming that CGA received enrollment paperwork? It was marked as delivered last Thursday, but haven't gotten anything confirming that CGA actually has it in hand & processed.
 
DS paperwork goes out tomorrow. Not sure we can expect any acknowledgements though...nothing on uniform/cadet contact forms that were emailed last week. Guessing the admin office has its hands full right now....!
 
I’m glad you posted this! Our enrollment forms that were mailed last week and were received in New London, per USPS tracking, on Monday. The forms that were emailed (medical and uniform measurements) were sent last week as well, and no email confirmation of receipt was received. Perhaps they confirmed receipt of the appointments due to the fact that it also had original ID documents and to let us know we won’t get them back until 7/1. I think in this regard no news is probably good news.
